Welcome to the Utah Division of Wildlife Resources Harvest Survey.

The Utah Division of Wildlife Resources (UDWR) conducts annual surveys of hunters in an effort to gain much needed information about the hunting of wildlife in Utah. These surveys help us determine harvest and hunting effort in Utah during the most recent hunting seasons, and will help us to better manage Utah’s wildlife resources.

If we have contacted you via email or telephone, you have been randomly selected to participate in a survey for UDWR. In most cases the survey takes only a few minutes to complete. Please participate in this survey even if you did not hunt or did not harvest, as this information also helps us determine the number of hunters who take advantage of hunting opportunities in Utah.

This is a voluntary harvest survey only. If you obtained a limited entry, CWMU, or once-in-a-lifetime permit in 2025, you will still need to file your mandatory harvest report online at wildlife.utah.gov or by calling (800) 221-0659, 7 days a week from 7:00 AM to 10:30 PM Mountain Time, excluding major holidays.

Survey is for selected hunters only. To begin, enter your Customer ID and Date of Birth and we will verify that you are eligible for a current UDWR survey. Your Customer ID can be located in the email we sent you inviting you to take this survey, and is also printed on your hunting license or permit.
